<p style="text-indent: 22pt; line-height: 200%"><font color="#333333">福布斯（Forbes）杂志最近以器官移植做为封面报导主题，就器官移植的需求及供应情形、地下交易中心、网路掮客行业等作了深入报导。
<P></P>中国在2000至2005年间估计有6万件器官移植，是之前6年移植总数的3倍。1999年从事肝器官移植的医院有22所，目前则已有逾500所。这些器官是从何而来？人权人士及前加拿大国会议员大卫•乔高（David Kilgour）以及人权律师大卫•麦塔斯（David Matas）于2006年7月共同提出的报告指出，为了满足国内器官移植中心的需求，中国隠密集中营（secretive gulags）不仅杀死犯人，而且也可能从犯人身上活体摘取器官，而大部分是来自于被迫害的法轮功成员。
<P>一个合理的解释是这些器官来自新近处死的犯人，而执行死刑的时间是按照医院需要来定的。就这一点就足以冒犯许多伦理学者。位于加拿大蒙特利尔“器官移植协会”（The Transplantation Society）的医疗主任德孟尼科（Francis Delmonico）医生说：“我们器官移植协会反对使用处死犯人的器官，因为在实施中如果没有透明度和监督，就会产生不断升高的器官需求驱使死刑增加的隐忧。”他说：“我知道有人在需要器官的前一周到中国去。那么，想想看，这是否说明他们需要在下周将一个人处死？”
<P>2005年7月中国卫生部副部长黄洁夫证实中国移植产业的器官来源于死刑犯，他并认为应该推动中国的相关立法工作。依据新的暂行法律规定，必须先取得该犯人的书面同意。去年秋天北京当局也全国监狱死刑的管控权收由中央管理。世界卫生组织的诺尔（Luc Noel）博士表示，这些变革措施的采行，已导致器官供应量的减少。
<P>由于器官银行无法满足大量器官移植的需求，使得网路掮客这个行业因运而生。科安（James Cohan）即是从事器官移植的掮客，他在网路上所列示的收费标准为：一个肾14万美元，一个心脏、肝或肺是29万美元，这些费用已包涵医院及开刀费用，以及一位随员的机票及住宿费用。
<P>科安的收费不低，而他所标榜的是器官的品质，因为他认为器官的品质比挑选医生还重要，而买方只要准备好钱，他就能快速提供新鲜的器官。科安在1990年后期曾因涉嫌贩卖器官在意大利被拘留了数个月，最后因检控方无法找到证据而被释放。
<P>他强调，他的所做所为都是合法的，他并不从事器官的买或卖，他只是将客户带到可以为他们从事器官移植的医院。目前他约每周就会有一位客户是来自于全球各地，而他是透过15个有往来的移植医院网络找寻适当的器官，这些医院坐落于中国、印度、菲律宾、南非、新加坡、巴基斯坦以及南美。
<P>至于科安所接的案件中，客户的满意程度如何，他们住哪里，开刀后活了多少年等问题，科安则不愿意详细回答。而事实上，的确有许多人已经在从事或想尝试做科安所做的事情。这些掮客利用网路，担任有钱国家濒临死亡的病人与开发中国家医院的中间人，而这些医院往往器官来源不明和道德规范不高。
<P>依据台湾立法院最近所举办公听会的数据，2005年台湾有787位完成器官移植的病人，其中有450人是在中国进行移植手术的。世界卫生组织统计，2004年全球完成肾移植有六万一千人，其中有10%系以移植旅游方式进行。诺尔博士即指出，近5年来器官移植旅游明显盛行。
<P>伊朗是目前全球唯一允许器官商业市场合法化的国家，对象仅限于其国民。另沙乌地阿拉伯也已在10月份通过一项法案，准许没有关系的捐赠者与受赠者间可有1万3千美元上限的金钱往来。
<P>大部分的国家采取规范严格的捐赠体系，如美国的联合器官分享网络（United Network of Organ Sharing）针对死亡捐赠者的每一个器官进行监控，问题是，过去10年来美国器官等待名单人数呈倍数成长，达九万四千人，而每年有列在等待名单上的一万二千名病人因等不及移植而死亡。一个幸运的肾受赠者平均必须等3.2年。在西欧，肾受赠者同样也要等3年，到了2010年等待期间还可能增加到长达10年之久。
<P>造成器官短缺的主要原因是车祸死亡率的降低，导致供应的减少；同时，寿命的延长以及药物的发达，也是导致需求增加的原因。
<P>依据器官观察（Organs Watch）的了解，肾的活体捐赠交易，在菲律宾是1千5百美元，摩尔多瓦及罗马尼亚是2千7百美元，土耳其7千5百美元，秘鲁1万美元，美国是3万美元。
<P>土耳其、印度及伊拉克曾经是最大的器官移植地下交易中心，但后来因政府的取缔或因战乱的发生，目前生意已大不如前。现在最热门的交易地方已转移到中国、巴基斯坦、巴西、哥伦比亚、蒙特内哥罗（Montenegro）、菲律宾及南非。<BR></P></font></td>
